최근 소아성폭력에 대한 관심의 증가에도 불구하고, 아직 국내에서는 성폭력피해 아동에 대한 표준화된 치료법이 확립되어 있지 않았다. 본 논문에서는 효과가 가장 잘 입증된 아동을 대상으로 한 외상집중 인지행동치료를 근간으로 저자들이 수정 보완하여 적용한 인지행동치료 사례를 보고하고자 한다. 성폭력을 당한 후 외상 후 스트레스 장애로 진단받은 2명의 여아에게 인지행동치료를 실시하였으며, 심리학적인 평가도구를 통해 치료전후 임상적인 변화를 평가하였다. 사례 A는 2년간 친부에 의해 만성적으로 성폭력을 경험한 여아로 사건을 폭로한 것 그리고 그 결과로 부모가 이혼을 하게 된 것에 대한 죄책감을 감소시키는데 초점을 두었다. 사례 B는 집에 무단 침입한 괴한에 의해 성폭행을 당한 경우로 다른 사람들의 반응에 대한 왜곡된 지각을 교정하는데 초점을 맞추었다. 심리학적 평가에서 두 아동 모두 우울과 불안뿐만 아니라 내면화 및 외현화 점수에서의 임상적인 호전을 보였다. 비록 두 사례 모두 치료초기에는 부정적적 사고에 대한 대안 사고를 찾고 노출작업을 하는데 특히 어려움이 있었으나, 이를 극복하고 성공적으로 인지행동치료를 적용할 수 있었다. 추후 보다 많은 사례들의 축적 및 비교연구를 통해 본 치료 프로그램의 효과가 입증되어져야 할 것이다.

In spite of the growing recognition of child sexual abuse, a standard treatment for sexually abused children has not yet been established in Korea. In this report, we introduce a modified individual cognitive behavioral therapy (CBT) program and illustrated two clinical cases applying our program. A standard 12-session CBT program was applied to two female children suffering post-traumatic stress disorder as a result of sexual abuse. The clinical changes were assessed by several psychological measures. Case A, offended by her biological father for 2 years, was focused to diminish guilty feelings about the disclosure and the resulting divorce of her parents. Case B, molested by the unacquainted man who broke into her house, was centered to correct distorted perceptions of others’ reactions. Psychological measures revealed clinical improvement in depression and anxiety as well as in internalizing and externalizing scores in both cases. This case report showed the successful engagement of our CBT program for sexually abuse children, although both cases, in particular, had difficulties in generating alternative positive thoughts and practicing gradual exposure. A comparison study with large sample size are needed to confirm the efficacy of this program.
