본 연구는 한국 여성의 초경 연령을 분석하여 변화 추세를 확인하고 초경 연령과 만성 비전염성 질환과의 연관을 확인하는데 연구의 목적이 있다. 연구방법과 관련하여 2016-2018년 국민건강영양조사 자료에서 20-49세 여성을 대상으로 초경 연령을 조기 초경(11세 이전), 평균 초경(12-15세), 늦은 초경(16-19세)으로 구분하여 인구사회학적 특성, 비만, 고혈압, 이상지질혈증, 당뇨 및 류마티스 관절염 등 만성 비전염성 질환과의 연관을 회귀분석으로 확인하였다. 연구결과, 20-49세 한국 여성의 초경 연령의 평균은 12.8±0.03세로 나타났다. 고혈압 유병은 조기 초경군에서 유의하게 높았고(OR=1.97, 95% CI=1.24-3.13, p=.004), 이상지질혈증일 때 높았다. 류마티스 관절염 유병은 조기 초경군에서 유의하게 낮았다(OR=0.07, 95% CI=0,01-0,49, p=.008). 본 연구는 대표성 있는 최신 대규모 국가자료를 활용하여 한국 여성의 초경 연령과 만성 비감염성 질환과의 관련성을 확인하였다는 점에 의의가 있으며, 향후 각 질환 유병에 영향을 미치는 관련 요인을 규명하는 후속연구가 진행되어야 할 것이다.
The aim of this study is to examine age at menarche of Korean women to check the trend and to identify the association with chronic non-inflammatory diseases. Using the 2016-2018 Korea national health nutrition survey, women aged 20-49 years were identified by regression analysis such as demographic characteristics, obesity, hypertension, dyslipidemia, diabetes and rheumatoid arthritis, by dividing the initial age into early AAM (under 11 years old), average AAM (12-15 years old), and late AAM (16-19 years). As a result, the mean age of AAM was 12.8±0.03 years. The prevalence of hypertension was significantly higher in the early AAM group (OR=1.97, 95% CI=1.24-3.13, p=.004), higher in dyslipidemia. The prevalence of rheumatoid arthritis was significantly lower in the early AAM group (OR=0.07, 95% CI=0,01-0,49, p= .008). This study is meaningful in confirming the relationship between AAM and non-communicable disease in Korean women using the latest nationally representative data, and follow-up studies to identify related factors affecting the prevalence of each disease should be conducted in the future.
